Senate Bill 12 Required Consent for School Based Related Health Service

To comply with Senate Bill 12, a parent or guardian must fill out the 2025-2026 School Based Related-Health Services Consent forms in Skyward Family Access indicating which services your student is allowed to receive, including physical and school counseling support. 

Please log in to Skyward Family Access to complete the required forms. Feel free to contact your campus counselor or nurse with questions about the health services available at your student's school.
