Below is a list of immunizations that are required in order for
students to attend school in the State of Texas:

DTP-Td - Diptheria-Tetanus-Pertussis
|
All students at least four doses (if seven years or older, only three
doses are required) including one on or after the 4th birthday.
A booster is required every ten years.

|
OPV-IPV - Polio
|
All students under eighteen years at least three doses of vaccine
including one on or after the 4th birthday.
|
|
Measles
|
All students at least one dose of vaccine on or after the 1st
birthday. In addition, all students age five years and older who
are born on or after September 2, 1991, must show proof of two doses of
vaccine (received on or after the 1st birthday) no later than
thirty days after their 12th birthday. Students born prior to
September 2, 1991, must show proof of two doses of vaccine (received on
or after the 1st birthday), no later than thirty days after their 12th
birthday.

Rubella
|
All students at least one dose of vaccine on or after the 1st
birthday.
|
|
Mumps
|
All students at least one dose of vaccine on or after the 1st
birthday.
|
|
Hepatitis
B:
|
Effective August 1, 1998 - three doses of Hepatitis B vaccine are
required for all students age 5 years and older who are born on or after
September 2, 1992. Effective August 1,
2000, children born between September 2, 1988 and September
1, 1992 (inclusive) must show proof of three doses of Hepatitis B vaccine.

Hib -
Haemophilis influenza type B
|
At least one dose of Hib vaccine for students 15 months through 4
years given on or after fifteen months of age (unless primary series and
booster have been given earlier.

|
Varicella
- Chickenpox
|
Effective August 1, 2000, children
born after September 2, 1994, must show proof of one dose of varicella
vaccine on or after 1st birthday. Children born between September 2,
1988 and September 1, 1994 (inclusive) must have one dose of varicella
vaccine (received on or after the 1st birthday (no later than 30 days
after their 12th birthday. If the first dose was given after age 13,
then 2 doses are required. If the student has had the chickenpox
disease a written statement from the parent or physician (including the
date) will be accepted.

Records must include the name of the vaccine given, the complete date
(month, day, year) and the physician's signature or clinic stamp.
